Attention to these multi‐level factors is necessary to accurately and comprehensively understand and intervene to improve human health. The population health framework is a valuable conceptual framework to guide nurse researchers in identifying and targeting the broad range of determinants of (...) health. However, attention to the intermediate processes linking multi‐level factors and use of appropriate multi‐level theory and research methodology is critical to utilizing the framework effectively. Nurse researchers are well equipped to undertake such investigations but need to consider a number of political, societal, professional and organizational barriers to do so. By fully embracing the population health framework, nurse researchers have the opportunity to explore the multi‐level influences on health and to develop, implement and evaluate interventions that target immediate needs, more distal factors and the intermediate processes that connect them. PURPOSE: To evaluate the effects of social support on comprehension and recall of consent form information in a study of Parkinson disease patients and their caregivers.DESIGN and METHODS: Comparison of comprehension and recall outcomes among participants who read and signed the consent form accompanied by a family member/friend versus those of participants who read and signed the consent form unaccompanied. Comprehension and recall of consent form information were measured at one week and one month respectively, using Part A of the (...) Quality of Informed Consent Questionnaire .RESULTS: The mean age of the sample of 143 participants was 71 years . Analysis of covariance was used to compare QuIC scores between the intervention group and control group . In the 1-week model, no statistically significant intervention effect was found . However, the intervention status by patient status interaction was statistically significant . In the 1-month model, no statistically significant intervention effect was found . Again, however, the intervention status by patient status interaction was statistically significant . At both time periods, intervention group patients scored higher on the QuIC than did intervention group caregivers, and control group patients scored lower on the QuIC than did control group caregivers.IMPLICATIONS: Social support played a significant role in enhancing comprehension and recall of consent form information among patients. The right to refuse medical intervention is well established, but it remains unclear how best to respect and exercise this right in life support. Contemporary ethical guidelines for critical care give ambiguous advice, largely because they focus on the moral equivalence of withdrawing and withholding care without confronting the very real differences regarding who is aware and informed of intervention options and how patient values are communicated and enacted. In withholding care, doctors typically withhold information about interventions judged too futile (...) to offer. They thus retain greater decision-making burden and face weaker obligations to secure consent from patients or proxies. In withdrawing care, there is a clearer imperative for the doctor to include patients in decisions, share information and secure consent, even when continued life support is deemed futile. How decisions to withhold and withdraw life support differ ethically in their implications for positive versus negative interpretations of patient autonomy, imperatives for consent, definitions of futility and the subjective evaluation of benefits and burdens of life support in critical care settings are explored. Professional reflection is required to respond to trends favouring a more positive interpretation of patient autonomy in the context of life support decisions in critical care. Both the bioethics and critical care communities should investigate the possibilities and limits of growing pressure for doctors to disclose their reasoning or seek patient consent when decisions to withhold life support are made. The Stance of Plato addresses Plato's particular fusion of literature and philosophy. Albert Cook examines a number of Plato's major dialogues to ascertain further the bearing of "rhetoric" and the dramatized dialogue on the relationship between literature and philosophy. Using an engaging and occasionally poetic style, Cook studies the implications of Plato's literary form and the historical context of his ideas. The Stance of Plato helps bridge the gap between scholars interested in Plato's arguments and logic and (...) those interested in their literary aspects, suggesting that literature and philosophy may not be separable domains. The book will be an important work for classicists, philosophers, and scholars of literature. (shrink)

In this paper we interpret a characterization of the Gödel speed-up phenomenon as providing support for the ‘Nagel-Newman thesis’ that human theorem recognizers differ from mechanical theorem recognizers in that the former do not seem to be limited by Gödel's incompleteness theorems whereas the latter do seem to be thus limited. However, we also maintain that (currently non-existent) programs which are open systems in that they continuously interact with, and are thus inseparable from, their environment, are not covered by the (...) above (or probably any other recursion-theoretic) argument. In four studies, we examine the “LIFE IS A JOURNEY” conceptual metaphor using as data output from semantic and episodic memory. In the first three studies output from semantic memory indicates that undergraduate samples, when primed to think in “LIFE” in terms of a course followed until one's 70th year, provided a set of events output in a sequential order and when compared to a second sample, showed high agreement on the ages in which the events would occur. These data (...) were taken as supportive of a “JOURNEY” metaphor in which one progressed along a life path. Study 2 indicated that the “LIFE” events produced also aroused specific subthemes, consistent with the notion of the arousal of lower-level “LIFE-JOURNEY” metaphors (e.g., “LOVE IS A JOURNEY”). In a third study, we experimentally constrained the order of report from semantic memory. These data indicate that free output of events (as in Study 1) involves both a forward-temporal output, and clustering of life themes, but that forcing a forward-temporal order is indicative of the arousal of the superordinate conceptual metaphor but not the lower-level themes. Finally, in an episodic memory task one finds the reverse, namely arousal of the lower-level themes but not of the superordinate conceptual metaphor. Taken together, these findings indicate conditions under which the “LIFE IS A JOURNEY” metaphor is facilitated, and when lower-level metaphors are made more salient. In a more general way, the studies provide a new tool with which to study conceptual metaphors. (shrink)

Providing efficient and clean power is a challenge for devices that range from the micro to macro in scale. Although there has been significant progress in the development of micro-, meso-, and macro-scale power supplies and technologies, realization of many devices is limited by the inability of power supplies to scale with the diminishing sizes of CMOS-based technology. Here, the authors provide an overview of piezoelectric energy harvesting technology along with a discussion of proof of concept devices, relevant governing equations, (...) and figures of merit. They present two case studies: (a) energy capture from the operation of a novel shear and elastic modulus indentation device subjected to applied voltage and (b) energy capture from vibrating commercial bimorph piezoelectric structures mounted on household appliances. Lastly, areas of development needed for realization of commercial energy harvesting devices are suggested. (shrink)

The purpose of this paper is to examine briefly what is meant by the term “philosophy” as it occurs in such expressions as “philosophy of physics,” “philosophy of mathematics,” “philosophy of science” and the like. The discussion will be divided into three parts: the first will consider several distinct meanings of the term “philosophy;” the second will discuss one of these in some detail, namely, philosophy as analysis; the third will seek to answer an important question concerning the results of (...) philosophic analysis. My purpose in this brief paper is to consider the implications of a radically different computer architecure to some fundamental problems in the foundations of Cognitive Science. More exactly, I wish to consider the ramifications of the 'Gödel-Minds-Machines' controversy of the late 1960s on a dynamically changing computer architecture which, I venture to suggest, is going to revolutionize which 'functions' of the human mind can and cannot be modelled by (non-human) computational automata. I will proceed on the presupposition that the (...) reader is familiar with some of the fundamentals of computational theory and mathematical logic. (shrink)

In recent years, philosophers have developed accounts of cognitive and affective scaffolding to describe the contribution of environmental resources to the realization of mental abilities. However, an integrative account, which captures scaffolding relations in general terms and across domains, is currently lacking. To close this gap, this paper proposes a pattern theory of scaffolding. According to this theory, the functional and causal role of an environmental resource for an individual agent or a group of agents concerning a mental ability in (...) a situational context can be specified by multiple conditions that a scaffold can satisfy. On this view, scaffolds can be described in terms of patterns of relevance-weighted, gradually satisfied scaffolding conditions. The resulting theory will be applied to cases of mathematical cognition to demonstrate its plausibility and feasibility. The pattern theory of scaffolding, it will be shown, has important implications for research on embodied, embedded, extended, and enactive (4E) cognition. (shrink)
